“My Dad Is…” The Ultimate Super Dad Questionnaire for Father’s Day

I created this fun download of The Ultimate Super Dad Questionnaire for Father’s Day! “My Dad is…” for my kids and you too!!!


My son brought home a “All About Mom” questionnaire a few weeks ago for Mother’s Day. I LOVED it! His teacher asked him questions about me and she wrote down, verbatim, his answers. They were too cute! 

Who knew I smelled like Cinnamon and was 15 years old?!?!?!?!?! 

His adorable answers inspired me to make a questionnaire for each of my boys to do for their dad this year for Father’s Day.

Daddy is a Super Hero.

My kids totally idolize their daddy. He is their ultimate hero. I love that they want to be like daddy!

Men who love their children and work at a being a good daddy are the sexiest men! 


I am so blessed that my boys have a great man to aspire to be like and my girls have a bar set very high for any men in their life.


Jon is rock hard with his boys, but put his girls in his arms and he is a puddle of goo. Yep. Just melted. 



The big boys were able to fill out the questionnaire on their own. The two little boys needed a little help with spelling and writing longer sentences. 

Open ended questions create the most hilarious conversations with children! I just loved their answers!

Father's Day Printable

Jordan guessed that Daddy was 19 and then wrote the number backwards so it now says he is 91!

Jimmy decided that Dad’s Super Power is that he can see through walls.

    This is such a great idea! I’ve been racking my brain trying to think of what to give my husband for Father’s Day since he’s not really into cutesy stuff. this is perfect!

    What a cute printout! If I was still teaching I would print it out for my students! I will let my daughters know about it so they can print it for their kids to do!

    All of these photos are so adorable! I love the tips. I always struggle with coming up with good ideas. This is perfect.
