This Valentine’s Day Printable Questionnaire is perfect for any child who wants to give a sweet card to someone they love.
The questionnaire was purposely designed to be used for any person you choose or multiple people. Mom, dad, auntie, uncle, best friend, teacher, brother or sister.
These types of questionnaires where the kids get to fill in the blanks to the statements are my absolute favorite! The key to these questionnaires is to write down the first word that comes to your child’s head. These are the BEST answers!
My middle son brought one of these questionnaires home from school for Mother’s Day when he was in preschool and my favorite line was, “My mom smells like cinnamon.” I am always in the kitchen cooking and baking. This answer made me so happy!
If your child is not old enough to write his answer, then you can fill in the blank for them. But if they are learning to write this is a fun activity for them to test their writing skills. I spell the words out for my Kindergarteners so they can write in the blanks themselves. If it is very messy or illegible, then I will go back and write to the side the answer as well.
My older kids know this drill and have so much fun coming up with great answers. They get really silly sometimes and sometimes extremely sweet. I love it when they are mix of both.
